Medical Information

Medical Information/Medication

If your child has a medical condition it is vital that you make school aware.  If your child requires routine medication whilst at school you will need to complete the form administering medication before it can be given.  A form can be obtained from the school office or downloaded. Permission to administer medicines. Only medicines prescribed by a Doctor and have a label on from the Pharmacy will be administered to your child upon completion of the form.

Asthma

If your child suffers from asthma, please inform school as soon as possible and an AM5 form will need to be completed.  The form can be obtained from the School office or downloaded. ALL children with asthma and using an inhaler must have a spare inhaler in school for their use. The school does have an emergency use inhaler and you will need to complete a consent form for your child allowing us to use the same. Please speak with the office team who will be able to provide you with this form. AM5 for asthma form 

EPI-PENS

If your child needs to use an Epi-Pen, school must be informed immediately. The school require 2 Epi-Pens – one for your child’s classroom and one to be kept in the office. The school does have an emergency use Epi-Pen and you will need to complete a consent form for your child allowing us to use the same. Please speak with the office team who will be able to provide you with this form.

ALLERGIES

In light of the recent news article regarding allergies and awareness in school we would like to remind all our parents to; 

Provide the school with sufficient and up-to-date information about your child’s medical needs using a standard form and be involved in the arrangements to develop and review your child’s individual healthcare plan;  

Deliver any medicine, which cannot be administered outside the school day, to the school in its original container(s) showing the pharmacy label and ensuring that the medicine is not out of date and that it has previously been stored correctly;  

Clearly mark all medicines with the following information; the child’s name on the medicine; when the medicine should be given; the prescribed dose and pharmacist’s instruction, e.g. after meals;   

Notify the school immediately (in writing) of any changes or alteration to a prescription or recommended treatment so that adjustment can be made to individual healthcare plans or previous agreement;  

Inform the school about other issues or symptoms your child may have experienced overnight or before school; this is particularly important for asthma conditions ; 

Ensure you or another nominated adult are contactable at all times;  

It must be remembered that the prime responsibility for a child’s health rests with parents or carers.

If your child has an allergy and you have not made the school aware, please come and speak with the office team who will provide you with a form to complete. Please bring medical evidence either from your GP or Hospital to support the same so the correct care plan/risk assessment can be put in place. 

If you have previously stated your child has an allergy but not returned the Caterlink/Allergy Form then your child will receive a safe meal from the school kitchen unless you provide them with a packed lunch.
